Selection Criteria

Up to 10 Practice Change Fellows will be selected annually through a national competition, based on their leadership potential and demonstrated ability to implement and sustain new geriatric services or aging-related programs. Applicants must hold a leadership role in a health care delivery organization, health-related institution, or community-based organization with direct responsibility for geriatric services or aging-related programs.

A Selection Committee comprised of nationally prominent experts in practice change will select the Practice Change Fellows. In their evaluations, Selection Committee members will consider the following factors:

Applications will be further judged and selected based on potential to exert a profound impact on care for older adults, commitment to self-directed and peer learning and improvement, and a strong professional record reflecting positions of increasing leadership responsibility.
